The Arizona Department of Revenue issues city transaction privilege tax licenses for cities that have entered into a joint collection program with the state. They are commonly referred to as Program Cities. The fees vary from city to city, and a listing of participating municipalities and fees is provided on the Arizona Joint Tax Application.
The Arizona Joint Tax Application (JT-1) form can be filed online or the form can be downloaded from Arizona Department of Revenue website. Answers to questions can be found on their website or by contacting them.
